OCTO Keeps disconnecting and connecting from USB

I have rebooted my PC as well as powering it off and removing mains power until all lights go off. I am still experiencing the issue.

I have not opened my PC that might have potentially moved. However I did open my computer and unplug and re-plug both ends of the USB cable and the problem persists.

Has my Octo died? Any tips to hopefully get it to recover?
